Eligibility:

• We provide diapers & wipes for children ages 0-4.

• ​Must be experiencing financial hardship

• Must be a resident of Fayette County or within a 45-mile radius

Required Documents:

• A Completed Needs Assessment

• Your Photo I.D. (I.D. must match information on the needs assessment)

• Proof of Medicaid Services

• Copy of last pay stub
