Eligibility:

• We provide diapers & wipes for children ages 0-4.

• Applicants must be mothers between the ages of 13 and 30-For participants under the age of 18, a guardian's signature is required.

• ​Must be experiencing financial hardship

• Must be a resident of Fayette County or within a 45-mile radius

Required Documents:

• A Completed Referral Form

• Your Photo I.D. (I.D. must match information on referral form)

• Proof of Medicaid Services
